You need to state whether the assets
were purchased in your name; whether you have assets
purchased under other names such a joint account or
company; and in each instance whether you purchased
your investments through us. You then need to save the
details you have entered and confirm that the details
are correct before printing the form.

Note that more than one form will
be printed if you entered that you have investments
in more than one name. If you have not placed your investments
through us, you will need information on your fund manager
statements to complete the form correctly.
Sign and either fax or post your My Net Wealth®
Registration Form to us.
Once we process your form we will send
you an email containing instructions and your My Net
Wealth® login details.
If you have not placed your investments
through us however, you can expect to receive the
email in approximately 10 working days as we need
to arrange with the fund managers to have your details
sent to us.
